We are seeking a Full time, Part time or PRN SNF Physical Therapist to join our team at Bear Creek Senior Living in Colorado Springs, CO.

Your Role

As a Physical Therapist at Bear Creek Senior Living, you’ll be a vital part of a compassionate team dedicated to enriching the lives of our residents. Your duties will include:

  • Providing direct patient care, evaluating functional needs and outcomes, and consulting with other specialists
  • Reviewing patients’ functional needs and adjusting care plans to ensure optimal patient outcomes
  • Designing and implementing individualized plans of care based on thorough assessments of potential development, incorporating participation by significant others whenever appropriate
  • Conducting therapy practice within defined standards of care
  • Directing the assistance of support staff to achieve patients’ functional goals
  • Communicating effectively with other professional and support staff to achieve positive patient outcomes
  • Practicing facility and departmental policies and procedures, including quality improvement, safety, environmental, and infection control standards
  • Documenting evaluations, treatment goals, and plans; regularly updating documentation and maintaining prescriptions and signatures
  • Enhancing professional growth and development of self and others through participation in educational programs, in-service meetings, and workshops

Qualifications

  • Graduate of an American Physical Therapy Association (APTA) approved physical therapy program
  • Current Physical Therapist license from the state of Colorado Board of Physical Therapy Examiners
  • Knowledge and skills to care for the physical and developmental needs of the population age groups that will be receiving services
  • At least one year of experience in a skilled nursing or rehabilitation setting preferred
  • Ability to remain calm in emergency or crisis situations
  • Strong interpersonal and communication skills

Colorado Equal Pay for Equal Work Act

Pursuant to the Colorado Equal Pay for Equal Work Act, the expected compensation range for this position is $40.00–$45.00 per hour. Actual compensation may vary based on experience, education, and other relevant factors.
