How you’ll make an impact in this roleNavigate the Patient Journey: Serve as a dedicated guide and advocate for patients throughout their entire continuum of care, ensuring they feel supported, informed, and empowered at every step.
Bridge Care Connections: Act as a vital liaison between management, physicians, community organizations, and insurance providers to foster seamless communication and collaborative care.
Champion Quality & Education: Coordinate targeted treatment efforts while championing continuous quality improvement initiatives and educational programs that elevate team performance and patient outcomes.
Drive Data-Informed Progress: Track key clinical outcomes and compile meaningful data to recommend innovative process improvements that directly enhance our delivery of care.
Advocate for Equal Access: Proactively connect uninsured and underinsured patients with critical referrals and community resources, ensuring no one falls through the cracks for follow-up care.
Licensure / Certification / Registration:
- Licensed Registered Nurse obtained prior to hire date or job transfer date required.
- Registered Nurse credentialed from the Oklahoma Board of Nursing required.
- BLS Provider preferred. American Heart Association or American Red Cross accepted.
Education:
- Diploma from an accredited school/college of nursing OR Required professional licensure at time of hire.
